ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical effects of Simo Decoction Oral Liquid for the treatment of gastrointestinal dysfunction after stable thoracolumbar fractures.</p><p><b>METHODS</b>From May 2005 to July 2008, 81 patients with stable thoracolumbar fractures were randomly divided into treatment group (41 cases) and control group (40 cases) according to a random digits table. The treatment group included 32 males and 9 females with an average age of (47.19 +/- 5.18) years old ranging from 21 to 55 years, and the course was from 1 to 45 hours with an average of (7.83 +/- 1.29) hours. The control group included 30 males and 10 females with an average age of (46.31 +/- 3.72) years ranging from 20 to 54 years,and the course was from 1.5 to 43 hours with an average of (8.15 +/- 1.63) hours. The treatment group were dealed with Simo Decoction Oral Liquid,and the control group with neostigmine for acupoint block in bilateral Foot-Three-Li. The recovery of gastrointestinal function and the first passage of gas by anus were compared.</p><p><b>RESULTS</b>The time of recovery of gastrointestinal function in treatment group (7.27 +/- 3.14) h was shorter than that in control group (10.12 +/- 3.62) h. The time of first passage of gas by anus in treatment group (15.39 +/- 13.70) h was significantly shorter than that in contral group (24.02 +/- 18.11) h. The total effective rate in treatment group was higher than that in control group.</p><p><b>CONCLUSION</b>Both the treatment group and the control group have clinical effects in treatment of the restoration of gastrointestinal dysfunction after the stable thoracolumbar fractures, but the treatment group has more remarkable therapeutic effect and less side effects.</p>

ABSTRACT

The treatment of severely traumatic patients was changing from total care treament to the damage control surgery, as a result in the inflammatory reaction caused by trauma, in which the inflammatory marks, such as interleukin-6 and serum procalcitonin in the blood increased, and caused hypothermia, acidosis, and disturbance of blood coagulation, and resulted in the acute respiratory distress syndrome and multiple organs failure. A long-term operation as the second hit made the disease worse. In the patients, the femoral fracture was treated with external fixator; the pelvic fracture was treated with external fixator, and the uncontrolled haemorrhage in the pelvis was treated through direct hemostasis, angiography and embolism of arteries, and the tamponade of pelvis; the purpose of treatment of spinal fracture was keeping the stability of spine, avoiding the secondary injury on the spinal cord. It must pay attention to the injury of the adjacent organs and infection in the opening spinal injury. The result of operation was better in the incomplete spinal cord injury.

ABSTRACT

Anterior lumbar interbody fusion (ALIF) has become a standard technique for interbody fusion since it was reported by O'Brien in 1983. Now it is used to treat many degenerative disease,such as lumbar discogenic pain, lumbar instability, lumbar spondylolisthesis and so on. In recent years, ALIF has developed greatly in clinical application. Minimal incision operation and laparoscopic anterior lumbar interbody fusion has been already performed, which made the operation less invasive. Many internal fixation instruments are used to improve the stability of ALIF. Interbody fusion devices were developed and the fusion rates enhanced greatly. At the same time, the complications of ALIF also has been realized. This paper overviews the current progress in the clinical application of ALIF.

Splint fixation is an external fixation system,composed of retainer, splint, paper pad and traction. Pressure under retainer is the power source of splint fixation in treatment of fractures. Now we have a review literature about the progress of type and biomechanics of fixation retainer of splint, to offer the scientific parameters for modern reform of fixation retainer of splint.

ABSTRACT

Objective To evaluate the clinical outcome of external bracket fixation in the treat- ment of complex tibia diaphysis fracture involving intra-articular fractures.Methods Forty-two cases of complex tibia diaphysis fracture with proximal and distal intra-articular fractures treated surgically in our hospital from January 1999 to January 2004 were analyzed.The complex tibia diaphysis fractures were categorized according to the AO classification as type C2 (multiple segments fracture) and type C3 (ir- regular fracture),proximal and distal intra-articular fractures in 23 and 19 cases,respectively.Definite operation was done within one week.Twenty-two cases were treated with simple external fixator,and 20 cases treated with screws and external fixator.Results All the 42 cases were followed-up regularly. According to AO evaluation of the knee and ankle joint movement,83% (35/42 cases) of the cases gained satisfactory functional outcome,14% (6/42 cases) had quite satisfactory results and 2% (1/42 case) had unsatisfactory functional outcome.Conclusion External bracket fixation can obtain outcome of relative length of the tibia and fibula,tube structure reconstruction,smoothness of the articular surface and the parallel and symmetric relation of knees and ankles for complex tibia diaphysis fracture with proxi- mal and distal intra-articular fracture.The arthritis resulting in pain in movement and restriction of func- tion is considered to be the most important factor affecting the joint function.Early functional exercise is important for best recovery of knee and ankle function.
